Personal Information
As part of providing a service to you, your practitioner may need to collect and record personal information from you that is relevant to your current situation. If so, this information will be a necessary part of the assessment and treatment. You do not have to give all your personal information, but if you don’t, this may mean the service may not be able to be provided to you.
Purpose of Collecting and Holding Information
Information may be gathered as part of your assessment, diagnosis and treatment. The information is retained in order to document what happens during sessions, and enables your practitioner to provide a relevant and informed service. The practitioner’s administrative staff may have access to demographic and referral information, however do not have access to any other confidential information.
Access to Client Information
At any stage you as a client are entitled to access to the information about you kept on file, unless the relevant legislation provides otherwise. Your practitioners may discuss with you appropriate forms of access.
Confidentiality
All personal information gathered by your practitioner during the provision of the service will remain confidential and secure except where:
- It is subpoenaed by a court; or
- Failure to disclose the information would place you or another at serious and imminent risk; or
- Your prior approval has been obtained to provide a written report to another professional or agency (eg. a GP or a lawyer) or discuss the material with another person (eg. a parent or employer); or
- You are claiming a Medicare rebate and therefore give permission for your practitioner to consult with your referring GP
or if disclosure is otherwise required or authorised by law. Disclosure of consented information may occur via phone, post, email, fax, in person or other secure method.
Fees
All practitioners set their own fees. Please talk to your individual practitioner about their fees.
Cancellation Policy
In case of cancellation or postponement, please provide as much notice as possible. Cancellations within 48 hours of the appointment (not including weekends and public holidays) incur a $50 cancellation fee. In case of cancellation within 24 hours, the full fee is due (please note that Medicare will not provide a rebate for cancellation fees).
